9/15 - ACTION REQUIRED - Federal Impact Aid survey forms distributed to sites the week of Sept. 25

The Federal Impact Aid survey form has a new look for fiscal year 2017-2018 (sample attached).  The form will be prepopulated with student information and printed on an 8 ½ x 11 sheet of paper.  The form will be printed with English on one side and Spanish on the other.  

The survey forms will arrive at your school site the week of September 25 along with a packet of information.  The survey forms will be sorted by classroom for elementary and by period 1 for secondary.  For FY 17/18 the survey date is October 4, 2017.  Please be sure to distribute the forms on or after this date and be sure to collect a survey form from each student at your school.
