Required: Complete Partnerships in Education End-of-Year Evaluation at www.sandi.net/partners/evaluation and electronically submit to the Community Relations Department by Friday, May 24, 2002.

Optional: Complete Annual Mentor Award Nomination Form and/or Exemplary Partnership Nomination Form and return to the Community Relations Department, Eugene Brucker Education Center, Room 2121 by Friday, May 24, 2002.

Brief Explanation:

The Community Relations Department is pleased to announce that the annual Partnerships in Education End-of-Year Evaluation is available to schools and their partners online at www.sandi.net/partners/evaluation beginning May 1, 2002. Please complete the online evaluation form and electronically submit it to the Community Relations Department by Friday, May 24, 2002. Schools need to complete only one evaluation for all of their partners. This year's evaluation has been revised based on feedback from the Partnerships in Education Advisory Board and a model evaluation developed by the National Association of Partners in Education (NAPE).

Business partners will be sent instructions on how to access and submit the online evaluation. You are highly encouraged to complete your evaluations together. If this is not possible, please share your evaluation with your partner(s) and print a copy from your Web browser before it is electronically submitted to the Community Relations Department. Once submitted, the school or partner cannot retrieve the information.

The data presented each year in our annual report is possible because of your diligence in completing the end-of-year evaluations. The annual report is widely used by schools and this office for recruitment and marketing purposes.

Also available online is the Annual Mentor Award Nomination form to recognize a student and a mentor who have established an outstanding classroom relationship, and the Exemplary Partnership Nomination Form. Schools wishing to submit nominations for the Annual Mentor Award and/or the Exemplary Partnership Award should print out the forms, complete them and send them to the Community Relations Department, Eugene Brucker Education Center, Room 2121 or by fax (619) 725-5599 by Friday, May 24, 2002.
