All forms required for submitting a case to SARB have been revised and can now be ordered from stock with the following stock numbers:
Letters A, B, C, D, the Attendance Process Log, and the Attendance Log Intervention Form are NCR forms for elementary and secondary schools with distribution requirements. Letters A, B, and D have attachments. Letter A requires a copy of the Education Code, which can be ordered as one page from the SARB Office and reproduced at the sites. The attachments for Letters B and D are an Attendance Report and Code Sheet, which can be generated at the sites. Letter D is new. Parent letters are available in five languages (Spanish, Cambodian, Hmong, Vietnamese, and Lao) from the SARB Office at (858) 490-8676. These letters are not NCR forms. They will require the sites to make copies according to the distribution requirements at the bottom of the letters. Many schools have produced their own parent letters, but these have not enforced SARB requirements. It is imperative that all schools use these sanctioned district attendance forms for uniformity. |
