Brief Explanation:

The annual medical/dental open enrollment period will be held November I through November 30, 2000. During this period eligible employees may enroll for medical, dental, and vision coverage if not currently enrolled, change enrollment to one of the other medical/dental plans being offered, and/or add eligible dependents not currently enrolled. All open enrollment changes become effective January 1, 2001.

It is very important that employees eligible for coverage read the accompanying information carefully and take timely action as appropriate. For your convenience, the open enrollment material may be accessed via the Employee Benefits web site: www.sandi.net/benefits. Also, open enrollment changes may be accomplished on-line or by following the procedure on page 3 and 4 of the attached materials.

Important: Please note that the Prudential HMO and POS medical plans are now offered as the Aetna Patriot V HMO and Aetna QPOS (Point of Service), respectively. If you are currently enrolled in a Prudential plan and are satisfied with your coverage, you do not need to complete a new enrollment form during this open enrollment period. All employees and dependents will automatically receive new Aetna cards at the end of December 2000.

Please remember that except as indicated in the "Additional Information" section on page 7 of the attached material, once this enrollment period is closed, you will not have the opportunity to make additional changes until the next open enrollment period scheduled for November 2001.
