Brief Explanation:

    The Finance Division is required to report to the Internal Revenue Service all Associated Student Body (ASB) payments for consultant services rendered between January 1 and December 31, 2000.  As a reminder, all pink copies of 2000 ASB consultant forms are now past due.  In addition, for secondary schools where payments are made to game officials, please submit pink copies of the Official Fee Request forms.

 Effective January 1, 2001, ASB consultant forms must be submitted within five days of making payments. The Finance Division is now required to provide this information to the Employment Development Department (EDD) within 20 days of payment. The EDD may assess a penalty of $24 for each instance of late filing. This penalty would be charged to the site, therefore it is important to submit your completed forms as soon as possible.

 Please make sure forms are filled out completely.  Pink carbon copies must be readable.  Social security number and dollar amount are very important.   The Internal Revenue Service requires a physical location for the address.  Post office boxes are not acceptable.
