Include the students/teams at your school that should be recognized, paying particular attention to the complete title of the honor received and the sponsoring agency/organization. Note the name of the teacher most directly responsible for the students achievement and encourage the teacher to attend if at all possible. Team awards must include names of all members in alphabetical order along with coaches/advisers and may be submitted on a separate page. (Note: Presentations to all CIF championship athletic teams must be coordinated through Barbara Brooks.) Duplicate the form or use additional sheets of paper as necessary. Confirmations will be sent.
