As part of our cooperative agreement with the
California Department of Education (CDE), and as a condition of accepting
funds for tobacco use prevention education, we are required to complete
the California Healthy Kids Survey (CHKS).
The CHKS surveys students statewide, and covers topics related to alcohol, tobacco, and other drugs. This is the first year it will be given in our elementary schools, although it has been done in Grades 7, 9, and 11. A small number of classes in Grade 5 will be randomly selected and the survey will be done between March 1 and March 16, 2001. The Intervention Services department staff will mail parent permission letters and administer the survey once parent permission is received. The surveys are anonymous and individual school results will not be
identified. Your site Life Skills teacher will provide you with a copy of
the survey for parents to preview and will be contacting you to schedule
