Brief Explanation:

The Partnerships in Education Evaluation Form will be distributed the week of May 7, 2001. Please complete the evaluation form and return to the Partnerships in Education Office, Room 2121, by Monday, June 4, 2001. Schools and district programs/departments will be provided with a listing of their current partners.

The business partners will also receive evaluation forms to complete. You are highly encouraged to complete your evaluations together. If this is not possible, please share your evaluation with your partner(s) and make a copy before mailing the original to the Partnerships in Education office. Based on your feedback, this evaluation form has been revised by the Partnerships in Education Advisory Board. This survey has been reduced from four to two pages, and schools need complete only one survey for all their partners.

Please note that if your school has new partnerships that were formally recognized at the special Board of Education meeting held on March 27, 2001, evaluations for these partnerships do not need to be submitted.

The data presented each year in our annual report is possible because of your diligence in completing the end-of-year evaluations. The annual report is widely used by schools and this office for recruitment and marketing purposes.

Also included in this mailing is a newly created mentor nomination form recognizing a student and a mentor who have established an outstanding classroom relationship. This award, sponsored by the Partnerships in Education Advisory Board, will recognize a K-6 and 7-12 success story. The awards will be presented at the fall partnership kick-off event.
