Brief Explanation: Principals of junior high and middle schools planning promotional programs or planning to distribute certificates of promotion are requested to complete the attached surveys and prepare appropriate attachments as outlined below: 1. Promotional Certificates – Order due March 28, 2001 To order promotional certificates, complete the "Certificates of Promotion" survey form (see Attachment 1). Submit the form along with a completed nonstock requisition (do not order on EFRA, this will be done for you); the budget charge number is 593-CW-4301-0421-xxx (school location number). Please order extra certificates to allow for a margin of error. 2. Promotional Certificate Covers – Order due March 28, 2001 Certificate covers are available at the cost of approximately $6.00 per package of 25. To order, complete a stock requisition using stock number 22-C-5920 and your school budget charge number. Please do not phone in your order and do not send your requisition directly to Purchasing. 3. Printed Programs A. Program Order – Due March 28, 2001 To order printed programs, complete the "Printed Programs" survey form (see Attachment 2). Submit the form along with a completed nonstock requisition with your school’s budget charge number. (Again, do not order through EFRA, this will be done for you.) In addition, camera-ready copy for the front cover of your commencement program should accompany your requisition. Please try to provide as accurate an account as possible so that costs can be projected correctly. Also, be sure to include an estimated cost for the programs, using last year’s cost as a guide. B. Class Roll – Due April 20, 2001 Unless otherwise specified, names will be printed alphabetically, last name first, in upper and lower case letters. If your list is presented in any other format, please include a note stating your preference for the actual printing. C. Balance of Program Copy – Due April 30, 2001 After submitting the balance of copy for printed programs, you will receive a copy for proofing and final revision as soon as it is available from the printer. Also attached is a checklist of materials due on March 28 (see Attachment 3). This form is included for your convenience; it is not necessary to return it.
