PSAT/NMSQT:

TESTING COORDINATOR'S CHECKLIST ADDENDUM

(To be used in conjunction with the Supervisor's Manual.)

 

Fall 2000 Testing Dates:

Tuesday, October 17 OR Saturday, October 21

Before Testing

___ 1.) Place announcements about PSAT testing in student bulletin.

___ 2.) Determine amount of additional student fees (if needed) to cover costs of proctors.

___ 3.) Obtain proctors and provide appropriate inservice.

___ 4.) Make arrangements (as needed) for students to take test at "alternate" test centers, if possible.

___ 5.) Verify you have received:

____ The number of test books and answer sheets ordered. (Verify count in each bag without opening the bag. Bags will contain sets of 5 or 25 items.)

____ The appropriate test form(s). (Check the form and date printed on test books and answer sheets. Refer to the Supervisor's Manual for specific information about the correct form and color for test materials.)

____ Supervisor's Manuals (1 per 35 test booklets).

____ Supervisor's Cover Sheet and Security Certificate.

____ Prepaid envelopes for return of answer sheets.

___ 6.) Maintain test materials in a secure location under lock and key.

After Testing

1.) Return the following to ETS immediately using the prepaid mailing envelopes:

a. all USED answer sheets,

b. all defective test books,

c. Supervisor's Irregularity Report (located at the back of the Supervisor's Manual and used only if an irregularity occurred),

d. SIGNED Supervisor's Cover Sheet and Security Certificate, and

e. the COMPLETED Fee Waiver Roster.

2.) No later than October 27, 2000, submit a copy of items c, d, and e in #1 above to Agnes Zufelt, Testing Unit, Annex 3.

3.) Direct your financial clerk to deposit fees collected FOR TESTS ONLY with district funds abated to Budget #473-AA-4305-0901.

4.) Destroy all UNUSED answer sheets.

5.) Store ALL USED and UNUSED test booklets until score reports arrive. Under certain circumstances, ETS may require that these be returned.
