Checklist for Families Leaving a School or School District

Notify your child's school as soon as possible.
Request a copy of your child's transcript from the school office.
Complete and sign the district's request to have an official copy of your child's transcript sent to the new school.
If your child has special needs, request a copy of the IEP from the school office or the Special Education Department.
Check with the principal regarding the latest date your child can leave and still receive full credit.
Check with the school office to see if your child can complete independent studies to complete credits.
Check with the school office to see if your child can complete credits after he/she has left the school district.
Check with the school office to see if classroom work can be provided for your child to finish through home schooling and receive credit to forward to the new district.
Check with the school office for classroom work that your child can complete during the transfer to keep current.
