Please download the NEW!!   2013-2014 Athletic Participation Packet
 
Complete entire form and turn in to your coach or Athletic Director, Danny Regas, prior to try-outs.  Athletes who play more than one sport will only need to have one packet completed for the entire school year.
NO OTHER PHYSICAL EXAM FORMS WILL BE ACCEPTED 

Coaches

Please download the  2012-2013 Coaches Packet   and turn in to Rosi Susalla, Mira Mesa Site Manager. She will process all staff and walk-on coaches.

Make sure you are up to date (and have turned in copies of)-

  1. Coaching Certification
  2. Concussion Class 
  3. CPR/ First Aid cards
  4. Walk-on coaches: TB Test
  5. Finger Printing at District Office
