Enrollment Requirements

All students must be registered by a parent and provide the documents outlined below. According to the district's No Shots, No School policy, students are not allowed to attend class until the school has received all required documents.
  • All students (Pre K–12)

    • Proof of residency. Can include items such as a utility bill, rental agreement, mortgage document, or military housing orders.

    • Proof of immunizations against polio, measles, mumps and rubella (MMR), diphtheria, tetanus and whooping cough (DTap), hepatitis B (Hep B) and chickenpox (Varicella or proof of having had chickenpox).
      - OR -
      A permanent medical exemption or personal beliefs exemption.

  • Preschool
    • Proof of immunizations against haemophilus influenzae type B (Hib)

  • Transitional Kindergarten 

    • Transitional Kindergarten Website

    • Required for all students who turn five (5) years between:
      • Nov.2 and Dec. 2, 2012 for the 2012-13 school year
      • Oct 2 and Dec 2, 2013 for the 2013-14 school year

    • Copy of a birth certificate, baptismal certificate or passport.

  • Kindergarten New to the district or attendance area
    • Children must be five (5) years or older on or before:
      • Nov. 1, 2012, in order to be eligible to attend Kindergarten for the 2012-13 school year.
      • Oct. 1, 2013,  in order to be eligible to attend Kindergarten for the 2013-14 school year

    • Copy of a birth certificate, baptismal certificate or passport.

  • All Grade 1 students

    • Proof of recent physical examination.

    • Must be at least 6 years old by October 1 unless they have completed a year of public school Kindergarten.

  • All Grade 7-12 students *New*

    • Proof of Tdap immunization against pertussis (whooping cough). Note: Tdap is not the same as DTap. Beginning July 1, 2011, AB 354 requires Tdap booster if not given on or after the 10th birthday. You may submit documentation over the summer. Include parent name and contact phone number or email on the shot record:
