(Td) if the last one is older than five years

is recommended

|
A chickenpox vaccine or
documentation of the disease is now required

Important Information
|

If you have personal
beliefs or religious convictions about immunizations, see your
school nurse. The school nurse can help you file an
exemption.

If the student's health insurance does not cover
the cost of vaccinations, then the required vaccinations can be given by the school nurse at
the school of attendance with the parent's/guardian's signed consent.

Please do not ignore the
law and just send your student for the first day of class. The schools will
be required to pay large fines if any students who have not completed
the requirements are allowed to attend class

Middle schools cannot give
a schedule to a student whose parent's/guardians have not met the
requirements of the law.

Please do not leave your
student sitting in the in school suspension area until this is
complete. We cannot make exceptions on the first day of school.
