Transcript Requests
Requests from adult former students can be made, either in person at Madison High School, or in writing. There are NO copies of a diploma. A transcript should suffice as proof of graduation. If mailing your request for a transcript, please send to Madison High School, Attn: Registrar. Include your name while at Madison, your birth date, the address you wish the transcript sent to, and $2 for postage and processing (cash,check, money order). Requests can be made by the “student” only, and will not be issued to a parent, spouse,or other relative. Colleges making requests must, either, have the student take care of having a transcript sent, or must have a release from the student, and include the $2 fee. Unofficial transcripts can be faxed school-to-school/business with a release at no cost. Feel free to call if you have any questions.
