California State law requires first graders to have a physical exam before December (Child Health and Disability Prevention exam) or your child will be excluded from school. You may sign a waiver if you do not want your child to have an exam. Kindergarteners are required to have an Oral Health Assessment (OHA) before May. Click the links below to print the required form.

To promote public health and safety, California state law requires students to receive certain immunizations at different ages in order to attend public school. Proof of immunization must be provided to your school at the time of registration to avoid any unneccessary exclusions from activities.
