2011-2012 District and Site Enrollment Forms

This page provides a link to the district and site enrollment forms that are needed for each student. A parent or guardian must complete and sign the required forms for students younger than age 18.
 
Please note: New students applying to iHigh should submit the online application and schedule an admissions interview prior to completing the enrollment forms. Click here to apply.
 
All students will need to provide proof of residence (utility bill, rental agreement, etc.). We also need to verify that the whooping cough vaccination (Tdap) has been given sometime after age 7, as per the new CA state law.
 
Students who are new to iHigh should bring the completed forms to the admissions interview. They will also need to provide a copy of the student's birth certificate and immunization record, as well as a copy of any out-of-district transcripts.

5) Request to Transfer Student Records  (used to request school records from previous school)
 
6) California Immunization Registry Form  (needed to upload immunization records)
 
7) School Lunch Information & Application (full price, reduced price and free lunch programs)
 
 
 
10) Home Language Survey  (only needed for students who are new to our district)
