2012-2013 District and Site Enrollment Forms

Click here to go to the online application, to apply for fulltime enrollment. New students applying to iHigh should submit the online application and schedule an admissions interview prior to completing the enrollment forms.

This page provides a link to the district and site enrollment forms that are needed for each student. A parent or guardian must complete and sign the required forms for students younger than age 18.

All students will need to provide proof of residence (utility bill, rental agreement, etc.). We also need to verify that the whooping cough vaccination (Tdap) has been given sometime after age 10, as per CA state law.

Students who are new to iHigh should bring the completed forms to the admissions interview. Srudents who are new to San Diego Unified will also need to provide a copy of the student's birth certificate and immunization record, as well as a copy of any out-of-district high school transcripts.
