Kearny High Athletic Participation Forms 2012-2013 *** Please Read All Directions *** Step 1 Athletic Physical Examination Forms These forms must go with you to your doctor. The Physician must fill out all pages; sign, date and stamp where indicated. The doctor's license number must be on the physical form. Completed Physical forms must be returned to the SIB office in Room 413. You will then receive the Participation Packet to complete with your parent(s) or legal guardian(s). Step 2 Athletic Participation Forms Complete the Athletic Participation Forms and return to the SIB Office in Room 413. Step 3 Insurance Under state law, school districts are required to ensure that all members of school athletic teams have accidental injury insurance that covers medical and hospital expenses. The insurance requirement can be met by the school district offering insurance or other health benefits that cover medical and hospital expenses (Education code Section 32221.5). Some pupils may qualify to enroll in no-cost or low-cost local, state, or federally sponsored health insurance programs. Step 4 Parent Meeting Once your grades have been checked, and one parent has attended an athletic parent meeting, you will receive a clearance. The first Athletic Parent Meeting is August 1, 2011, at 5:45 pm in the Kearny Auditorium. You may not practice, compete, or receive a uniform until all forms have been completed and a parent or guardian has attended an Athletic Parent meeting. Each coach has access to the eligability database. Make sure your packets are complete, or you will not be cleared. Check all boxes... Leave nothing blank!
