- High school transcript
- Immunizations including Pertussis Immunization

Proof of immunizations against polio, measles, mumps and rubella (MMR), diphtheria, tetanus and whooping cough (DTap), hepatitis B (Hep B) and chickenpox (Varicella or proof of having had chickenpox).
- OR -
A permanent medical exemption or personal beliefs exemption.

Proof of residency. The California Department of Education requires all schools to annually verify and have on file documentation confirming the address of all students enrolled in their school. (California Education code 48200:Title V, Section 432). This information will not be given to any agency or organization. Proof can include items such as a utility bill, rental agreement, mortgage document, or military housing orders.

Facts for Parents:

Facts for Parents is a yearly publication that contains basic information about San Diego Unified, explains parent rights and responsibilities, and information required by state or federal law to provide to all parents or guardians. It also contains important information, including the district Anti-bullying, Zero Tolerance, and Technology policies. Parents and students are required to sign a form confirming that they have read Facts for Parents and return the form to their school site.

Proof of Tdap immunization against pertussis (whooping cough). Note: Tdap is not the same as DTap. Beginning July 1, 2011, AB 354 requires Tdap booster if not given on or after the 10th birthday. You may submit documentation over the summer. Include parent name and contact phone number or email on the shot record:

Hoover Health Clinic
Hoover Health Center and La Maestra Health Centers are working together to provide health care services to hoover students. This partnership provides care for your child(ren) in two ways:School Nurses and La
Maestra Health Center Satellite Clinic. School nurses are available every during school hours. All visits to the school nurse are free of charge.
- Information and Consent Forms
HIV/AIDS Education:
HIV/AIDS education is included in sex education programs in grades 6, 8 and 10 or 11. Additional instruction is provided in grades 9, 11 and 12 by HIV program staff and representatives from community agencies. All HIV/AIDS instructional material is available for review at sex education parent meetings. After reviewing materials, parents can notify the school in writing if they do not want their child to participate in the lessons.
Free& Reduced-Price Meal:
The 2012-2013 Free& Reduced-Price Meal Application will be available on-line about mid-August(exact date is not yet available). The application can be completed by going to www.sandi.net and selecting the School meal Info/Application Shortcut.
