For students coming from within San Diego City Schools, the office needs a copy of your proof of residency only. Immunization Requirements for School Entry: POLIO 4 doses (3 doses are enough if at least one given after the 4th birthday) MMR 2 doses for Kindergarten entry HEP-B 3 doses for Kindergarten entry VARICELLA or proof of disease on a shot record (required for Kindergarten students and students coming from outside California) Eligibility: Kindergarten students born on or before November 1, 2007 are eligible for enrollment. Emergencies: When completing the enrollment form, it is very important that the name, address and telephone number of the person(s) to call in case of an emergency is included. Enrollment Options ProgramsSchool Choice
The Choice Program provides students with the opportunity to attend a school other than their neighborhood school. Students may apply to any school within the School District, with the exception of Magnet schools. No transportation is offered through this program and enrollment is on a space-available basis.
