Medication Forms

IMPORTANT:

Over the counter medications cannot be administered by anyone in the office other than the nurse unless a Physician's Recommendations for Medication form has been completed by both parent and doctor.  If this is not possible, a parent must come to the school to give the student the medicine.  For the school year 08-09 there is a nurse on-site two days a week.

Physician's Recommendations for Medication 
            This form is to be completed and given to the school nurse should your child require taking doctor prescribed long term and over the counter medication at school.  Medication will be administered by designated school personnel.  This form requires the physician and parent/guardian's signatures.

Authorization for Asthma Medication
            This form is to be completed and signed by both parent and doctor and provided to the school nurse, along with the medication.   Medication will be administered by designated school personnel.
