Elementary School Enrollment Requirements

All kindergarteners and students new to the San Diego City School District must provide:

1. Original shot records

2. Birth certificate

3. Proof of address (one of the following)
       Photocopy of a recent utility bill (gas, electricity, cable, phone)
       Photocopy of a rental agreement
       Photocopy of mortgage documents, deed to house or tax bill payment receipts
       Photocopy of military housing order

4. Copy of I.E.P. if applicable

If you cannot locate your child's birth certificate you can request it on-line at one of the following websites.  This information is provided for parental convenience.   The San Diego City School District does not recommend any particular website.

Kindergarteners 

• Must turn 5 years old by December 2 of the current year to enroll

• Must have all immunizations required for school entry completed (see below)

  POLIO	4 doses but:	3 doses meet requirement for ages 4-6 if at least one was given on or after the 4th birthday.
  DTP (Diphtheria, Tetanus, Pertussis)	5 doses but:	4 doses meet requirements for ages 4-6 if at least one was on or after the 4th birthday.
  MMR (Measles, Mumps, Rubella)	2 doses	Both on or after 1st birthday.
  Hepatitis B	3 doses
  Varicella		1 dose or health care provider-documented varicella disease (chickenpox) or immunity (had disease).
