Office Hours: Monday - Friday 8:00 a.m. to 3:30 p.m.
Application Requirements:
Are you interested in earning extra money? Would you like a free lunch? How about a job that doesn't interfere with your classroom schedule?
You must have at least a "C" grade average and have a good attendance record.
How to apply
Make an appointment to talk with the Food Services Area Manager or Food Services Manager in charge of the cafeteria at your school. Arrange with him/her to obtain a Sanitation Study Packet and take the test on food handling and sanitation practices. You'll also receive a processing packet that includes an employment application, employee's withholding allowance certificate, employee identification card, drug-free workplace form and the oath of allegiance form.
See school counselor to obtain a "Request for Work Permit."
Arrange for a Social Security Card if you do not already have one.
Completing the application process: Fill out and sign the following forms:
Employment Application; be sure to include your date of birth.
Employee's Withholding Allowance Certificate (Fill out & Sign)
Employee Identification Card (Fill out & Sign)
Drug Free Workplace Form (Sign)
Oath of Allegiance Form (Sign)
Employment Eligibility Verification Form (I-9) (Fill out & Sign- Section 1 only)
In addition to the forms, you will also need to submit the following
Pink Copy of Work Permit Request
Photocopy of School ID Card and/or INS Card
Photocopy of Social Security Card
Return completed packet and copies to the Food Services Manager.
